EVENT & STREET CLOSURE REQUEST FORM

Use this form to request a street closure, a parade request, or for special events. 
All requests MUST BE MADE AT LEAST 60 DAYS prior to event for review by City Administration.
Your application request does not confirm street closures or approval for parade/event.
An approval process will follow and the final determination will be forwarded to you.
It is important that all contact information is correct. 


This does not include city trail systems – please identify if you are requesting use or closure of any part of trail systems – we will forward these requests to Portland Parks & Recreation. 


ONE FORM MUST BE COMPLETED FOR EACH EVENT
COMPLETED FORMS MUST BE DOWNLOADED OR PRINTED BY APPLICANT AND SUBMITTED EITHER BY:
